Use this form to also let us know about things like donated items for students if you would like a tax receipt from ISI's Home Office.  Please include all of your contact information along with a list of items that were donated.  We will forward this to ISI's Home Office and you should receive a receipt within a few weeks.  Please let us know if you do not.
