This plan has a 45 day satisfaction guarantee and a lifetime no rate increase guarantee Assurant Employee Benefits The Secure Choice individual plan provides dental benefits with prepayment fees. To receive the benefits with this plan you select a Plan Dentist for you and your family members from the list of Plan Dentist. You may choose a different dentist for each family member. There are no deductibles, no claim forms, no annual maximums. There is a fixed copayment schedule for Plan Dentists, reduced fees on orthodontic procedures, no referral required for Specialist benefits, and there are benefits for pre-existing dental conditions. With this insurance plan you are able to choose any dentist you want to use. There is immediate coverage for preventive, but there are waiting periods for some procedures. Benefits cover up to $1000 annually for every covered family member, and $1000 lifetime orthodontia benefits.
